Description

The transition home after a hospital stay may feel overwhelming for a patient and their caregiver. Seniors' Resource Center (SRC) offers a bundle of services to assist you during this time in an affordable way. Our goal is to improve your quality of life while avoiding an unnecessary hospital readmission. We will listen to your unique needs and connect you with the individualized services that will assist your recovery at home. 

Is this Service for Me?

Are you 60 years old of older? This service has been designed around the needs of older adults.

Will you or someone you know need help with transportation, meals, housekeeping or other tasks once you come home from the hospital? We can help provide support for non-medical needs that are crucial to getting back to good health.

Have you been to the ER multiple times or had to be re-admitted for the same or related health concerns? Sometimes healing can be complicated. 

How it Works:

Step 1: Contact us at 303-235-6923 as soon as you know that you will be in the hospital or during your hospital stay.

Step 2: A Case Manager will visit you during your hospital stay to support you and begin arranging services for your care following your hospital stay. 

Step 3: A Care Manager will continue to support you and arrange services in your home as you recover.
