Program Description

Community Connections' mission is to create opportunities for children and adults with intellectual and developmental disabilities to lead healthy and fulfilling lives within our community.

Programs Available

Children Programs: Early Intervention / Family Support .
Adult Programs: Residential Program / Day Program / Supported Living Services Program / Vocational Services.

Ages Served

Children and adult programs, depending on Medicaid Waiver eligibility

Durango and surrounding areas

Disability/Needs Served

All Special needs, Must meet state criteria for Developmental/Intellectual disability plus Children’s Extensive Support eligibility for children under 18.
